This is the original problem from an employee...
What do you do if you hurt yourself on the job and the person whom was with you tells human resources they don't remember if the person who got hurt told them about being hurt; even if she was the one who was with the person that got hurt at the time the person got hurt. What if your human resources for three month didn't properly look up the information on the day in question. Then later on, they tell the hurt person that she was right about the day in question. Now all this time from the start was go turn down the request for workman compensation.
