This is the original problem from an employee...
I sent an email to an employee requesting the status of three physicians. When sent the email, I wasn't sure of the spelling of one doctors name and put a (?) after the last name. After I received the response, I cut and pasted the original information in the email and after being assured of the spelling of the last name, I removed the (?). Also, I was working on a number of documents at the time and inadvertently changed the date of the original email from Tuesday to Monday (thinking I was typing this into the body of the email) but the time was not changed from the original email and actually reflected a time 3 HOURS EARLIER THAN THE REQUEST FOR THE INFORMATION WAS ORIGINALLY MADE. There was no priority on the information needed and no lose of benefit or profit. Yet, I was terminated for this issue based on falsifying documents which my manager thought was deliberate. There had been no other work infractions, warnings or disciplinary actions.

That's pretty harsh, but I suppose it happens. They were probably looking for a reason to fire you.
