This is the original problem from an employee...
I did a drug round for afternoon, discovered that morning boxes of meds not sign but documented in notes as given, pt said did not receive meds. administer the meds and sign in the morning boxes and also initial time of administation under signature. Another colleague repeated the same meds 2hours later. When the colleague was confronted with the error she claimed not to have seen the times at the time she administer the meds. I was suspended with allegation of falsification of records. Because my colleague claimed I have signed in th morning boxes when I returned to the pt and realised that another dose of the same drug have been given.They also said that the previouse colleague that handed over to me gave evidence that she administer all the morning drugs and sign, so I have sign inside hers.
